A home-grown agency built around one idea: dignity isn't optional.

Dignity is the trade name under which American Trusted Nurses LLC operates its Colorado Program Approved Service Agency, supporting individuals enrolled in the HCBS-DD, HCBS-SLS, and HCBS-CES waivers.

We believe the people we support should set the terms of their own lives — where they live, how they spend their day, and who they spend it with. Our role is to make that possible, not to manage it for them.

Every service is delivered against a person's own plan — never a one-size schedule. Here's what we offer today.

Host Home
A Host Home Provider shares their residence with one to three individuals, offering daily support, habilitation, and a genuine household to belong to.
Adults
Supported Community Connections
One-on-one or small-group support to build relationships, skills, and independence through volunteering, recreation, and community life.
18+
Homemaker Services
Routine housekeeping and, where needed, extraordinary cleaning to keep a home safe and livable.
All ages
Community Connector
One-on-one support helping a young person build friendships and confidence out in the community they already live in.
12–17
Mentorship
Real-world coaching in self-advocacy — from interviewing your own providers to navigating a hard health decision.
18+
Respite
Planned, trusted relief care so a family caregiver can rest, travel, or simply take a breath.

Youth Day Services
Supervised daytime care for a young person whose disability — not just their age — means they need support while a caregiver works.
12–17
Prevocational Services
Building the everyday work-readiness habits — attendance, focus, teamwork, safety — that open the door to future paid work.

We follow the federal HCBS Settings Rule not as a compliance checkbox but because it describes what we already believe.

What to eat, when to wake up, who to spend time with — these stay the person's decisions, always.

In the community, not apart from it

Our services happen in real neighborhoods, workplaces, and public spaces — never in isolation.

Rights aren't negotiable

Any limit on a person's rights has to be specific, necessary, documented, and reviewed — never routine.

REFERRALS & PLACEMENT

Looking for a home for someone you support?

Case managers, families, guardians and individuals can refer to DIGNITY for Host Home and other HCBS services. We serve people enrolled in the HCBS-DD, HCBS-SLS and HCBS-CES waivers in Denver, Aurora and Adams County.

How a placement works

  1. Reach outEmail us with the person's first name or initials, waiver, county and the service you're looking for. Please leave out health details in the first message.
  2. Intake conversationWe call within two business days to talk through needs, preferences and timing, and set up a secure way to share records.
  3. MatchingWe review the referral packet and suggest a Host Home Provider whose home, routine and skills fit the person.
  4. Meet the homeThe person visits, meets the provider and decides. No one moves anywhere they haven't chosen.
  5. Transition planWith the case manager and support team, we plan the move, update the PCSP and complete the residential agreement before move-in.

What to have ready

  • Current Person-Centered Support Plan (PCSP)
  • Waiver type and approved services
  • Support level and most recent assessment
  • Guardian or authorized representative contact, if any
  • Medication list and health care providers
  • Behavior support plan or safety plan, if any
  • Any current rights modifications
  • Preferred move-in date and location

Records are shared only after consent from the person or their guardian, through a secure method we agree on.
